| dc.description | This is a shortened version of my doctoral dissertation at the University of Bremen. I thank my supervisors Michael Windzio, Olaf Groh-Samberg and Mark Taylor for their advice and support. They patiently answered all my questions and made myriads of constructive suggestions.
